hi,
you can call up/ mail ( whichever be the formal procedure) your boss and ask him ” boss so and so from so and place would like to get an appointment with you to discuss about….”
But before calling up your boss get important details of the person like his full name, co., purpose and see if he has any preferred time for appointment…
